At Saint Francis Adult Day Health, we provide a structured, medical-model day program designed to support adults with complex health needs, while helping families maintain stability at home.
Our program is ideal for individuals who need more than companion care or adult supervision. We serve participants living with memory loss, chronic illness, physical disabilities, or cognitive challenges, offering skilled support in a safe, social setting that allows them to remain in the community.
We offer a full range of clinical and supportive services, including:
On-site RNs and CNAs to monitor health, administer medications, and assist with medical needs throughout the day
Assistance with Activities of Daily Living (ADLs) such as bathing, toileting, ambulation, and grooming
Therapy services, including physical, occupational, and speech therapy, are all available on-site
Social services and care coordination to help families navigate changing needs, resources, and long-term planning
Dementia-certified staff trained to support memory-impaired participants with passion and dignity
Nutritious meals and snacks, including breakfast and lunch, daily
Van transportation to and from the program, helping ensure regular attendance and easing family burden
A full calendar of life enrichment activities—from music, art, and cooking to field trips, current events, and exercise
Opportunities for community engagement and connection, reducing isolation and promoting emotional well-being
While participants are with us during the day, families can work, rest, or attend to other responsibilities, knowing their loved one is in a medically supported, welcoming environment. Our goal is to meet participants where they are—physically, emotionally, and socially—and help
